Permission to Receive Call-Outs Required if You Change Service Providers

Hello Parent/Guardian,

Due to Federal regulations related to the Do Not Call List our call out system will automatically DEACTIVATE your phone number if you switch carriers or service providers.  Please notify the office if you change service providers whether your phone number transfers with you or not.  If you receive a new phone number it is important to let us know that as well.  We will need to reactivate any phone number that transfers to your new provider in-order for you to receive outreach calls.  Thank you for your attention to this.

If you have changed service providers, please provide your student’s name and the new phone number to be added back to the call-out list. You may use this form if you wish: Notification of change of service.
